When ordering, please provide the following information:

  • Full name of the child
  • Date of birth
  • Place of birth
  • Father’s full name (if recorded at the time of registration)
  • Father’s place of birth and occupation*
  • Mother’s full name
  • Mother’s place of birth, maiden surname, and occupation*
  • Certificate reference number*
  • Nationality at birth (if the event was registered overseas)
